Implemented – Case Study: Salesforce Health Services Modernization
The Problem:
- Facing the COVID-19 Pandemic, our client was in need of a testing notification system and contract tracing solution
- Current EHR lacked SMS capabilities and the ability to view lab results online using secure methods
- Disease Surveillance and Mapping tools were limited and prevented leadership from viewing outbreaks in real-time
- Lack of visibility between departments and limited opportunities for identifying critical patient needs
- Urgent need to investigate outbreaks and prioritize contact tracing to limit the spread of COVID-19.
The Solution:
- Using MuleSoft, Health Cloud was integrated with Cerner (EHR) to allow for Patient Demographics and Lab Test Results to be securely transmitted to Salesforce
- Using Experience and Marketing Cloud, developed a Rapid SMS and Email Notification System with a Secure HIPAA compliant landing page for patients to view their test results online
- Automated workflows to support patients who tested positive with guidance and next steps on how to isolate and prevent further infections
- ArcGIS Mapping using Salesforce Maps to visualize heat maps of outbreaks
- Intelligent Routing of Patients based on severity and underlying health conditions
The Results:
- Real-time COVID-19 Dashboard and Metrics to keep leadership informed. This allowed medical centers to be operationally ready when surges were expected
- Automated Patient Journey with templated health plans and checkpoints to allow providers to handle surges more efficiently
- A more robust PHI Data Model that displays patient records, lab test results, case worker interactions, and provider notes under one centralized window
- Re-usable API Asset with Cerner (EHR) allowed for scalability to enable additional use cases such as the Flu and RSV.
